CT demonstrates extensive pulmonary fibrosis in the mid and lower zones (note the extensive honeycombing) with co-existing severe centrilobular emphysema in the upper zones.

The fibrosis is attributable to the patient's known rheumatoid arthritis, and the emphysema to a long history of smoking.
