In the right lower lobe there is a cystic mass measuring 8.5 x 8.2 x 11.7 cm, containing air and layered curvilinear soft tissue attenuation structures. Consolidation and atelectasis in the adjacent lung. Moderate right pleural effusion.

Case Discussion

Typical appearance of ruptured pulmonary hydatid cyst. The layered curvilinear structures are detached endocyst membrane, (serpent sign).

Hydatid cysts can rupture into a main bronchus, lung parenchyma or pleural cavity. The rupture of a cyst into the parenchyma may cause consolidation around the cyst. Endobronchial spread of hydatid cysts are seen as centrilobular opacities.
