Rupture of a large left lung cyst with layering of the membranes and air-fluid levels associated with pleural thickening and enhancement. There is a near complete collapse of the left lung and contralateral shift of cardio mediastinum.

Case Discussion

Typical findings of ruptured pulmonary hydatid cyst in this case: layering of membranes and giving rise to Water-lily sign due to detachment of endocyst from pericyst.

In this case, there is also a rupture into the pleural cavity, giving rise to large tension hydropneumothorax with contralateral cardiomediastinal shift and requiring urgent decompression tube placement.
