Sacral insufficiency fracture

Presentation

Low backache, leg weakness and numbness bilaterally. Not able to bear weight on left side.

Patient Data

Age: 25 years
Gender: Female
mri

Increased STIR signals involving the marrow surrounding the right S1 and S2 and left S2 sacral foramina are noted with an incomplete fracture line at right S1 sacral foramen. There is no associated soft tissue lesion or distortion of the sacral margins. No involvement of the nerve roots. Normal sacroiliac joints.

Marrow edema of left femoral neck is also noted with joint effusion.

Case Discussion

Sacral insufficiency fracture is a subtype of stress fractures, which is due to normal stress on abnormal bone, most frequently seen in the setting of osteoporosis.
