Endplate sclerosis and irregularity involving both sacroiliac joints, more evident at the iliac sides. No joint ankylosis.
Case Discussion
Patient is known case of rheumatoid arthritis.
Radiological features are most likely in keeping with bilateral symmetrical sacroiliitis (grade III according to New York criteria).
The differential diagnoses for bilateral symmetrical sacroiliitis include: ankylosing spondylitis, rheumatoid arthritis, enteropathic arthritis, multicentric reticulohistiocytosis, and whipple disease.
