Sacroiliitis

Diagnosis almost certain

Presentation

Acute onset, non-traumatic low back pain, and difficulty in walking.

Patient Data

Age: 25 years
Gender: Female
mri

Ill-defined STIR hyperintense marrow edema with T1 hypointense bony erosion is noted involving bilateral sacroiliac joints predominately involving the lower third of sacral and iliac surfaces. No joint effusion or bony ankylosis. Findings are suggestive of inflammatory sacroiliitis.

Case Discussion

The patient initially came for a lumbar spine MRI which was normal however screening of the sacroiliac joints revealed changes of sacroiliitis. The previous radiograph was unremarkable. RA factor was negative. The patient was treated symptomatically with NSAIDs and physiotherapy.
