Presentation
Acute left-sided pelvic pain and sciatica with limited pelvic motions. CBC shows leukocytosis with elevated ESR.

The MRI sequences demonstrate a subchondral bone marrow edema of the surfaces of the left sacroiliac joint of low signal on TWI, high signal on T2WI with prominent enhancement, and joint effusion.
Edematous changes in the deep part of the ipsilateral iliopsoas muscle without evidence of abscess formation are depicted.
Case Discussion
MRI findings are consistent with left-sided acute sacroiliitis, the cause is not known in this case.
