Schwannoma of the tibial nerve

Diagnosis probable

Presentation

Right feet numbness.

Patient Data

Age: 50 years
Gender: Male

Right leg and knee joint

Well defined ovoid lesion, measuring 27 x 28 x 36 mm (AP x TD x CC) located eccentrically along right tibial nerve 37 mm proximal to knee joint, in intermuscular planes between biceps femoris and semimembranosus muscles.  Possible areas of necrosis are seen within the lesion. 

Lesion is abutting peroneal nerve at its posterolateral aspects and mildly compressing popliteal vein at anterior aspects.  Visualized peroneal nerve shows normal caliber and signal intensity. 

Note is made of osteoarthritic changes in right knee joint. 

No obvious cortical breach / marrow infiltration of tibia / fibula seen. No collection / mass lesion seen in myofascial planes of calf. No obvious abnormal hyperintensity / edema seen in visualized muscles.

Case Discussion

Overall above findings are suggestive of a neurogenic lesion of the tibial nerve, likely schwannoma
