Scrotal hematoma

Presentation

Patient underwent right-sided sperm retrieval procedure several weeks ago. Shortly thereafter, he began to experience testicular swelling. He is now experiencing worsening testicular swelling and pain on physical exertion.

Patient Data

Age: 30 years
Gender: Male
ultrasound

There is a heterogeneous collection within the right testis, without internal flow on color or power Doppler. There is a peripheral echogenic rim surrounding the collection with diffusely increased Doppler flow that is favored to represent testicular tissue. Normal arterial and venous waveforms are identified within this testicular tissue. There is a small hydrocele. There are additional mixed echogenicity extra-testicular collections.

Case Discussion

This is a case of an intratesticular hematoma, especially given the history of prior surgical intervention. Given the marked peripheral color Doppler flow, there was a concern for a possible superinfected hematoma. The patient was seen by urology who recommended the patient start oral antibiotics for two weeks. Follow-up urologic visit and testicular and scrotal ultrasound showed clinical and radiologic improvement.
