Second branchial cleft cyst - type II

Diagnosis probable

Presentation

Left-sided neck painless swelling for a few months.

Patient Data

Age: 20 years
Gender: Male

There is a definable, hypodense lesion involving a left-sided retro-mandibular region with a thin, peripherally enhancing wall between the sternocleidomastoid muscle and great vessels without CT detectable invasion. The overlying skin is intact.

The rest of the imaged organs appear unremarkable.

Case Discussion

CT findings are most likely suggestive of the second branchial cleft cyst ( type - II ) as described above given the typical location and young age. However, the most important differential diagnosis to consider is cystic nodal metastasis in adult patients.
