Primary infertility.

Patient Data

Age: 35 years
Gender: Female

Fluoroscopy

There are two separate fusiform endometrial cavities with one cervical canal. Each endometrial cavity ends in a solitary fallopian tube.

Both fallopian tubes are visualized and normal spillage of contrast noted at both fimbrial ends.

There are two uterine cavities with a flat contour of the fundus, separated by a hypointense septum that extends to the cervix.

Case Discussion

Features on HSG and MRI are consistent with a complete septate uterus, which is a common type of congenital uterine anomaly, and it may lead to an increased rate of infertility.

The shape of the external uterine contour is crucial to differentiate a septate uterus from a bicornuate uterus because the clinical and interventional approaches differ widely for each anomaly. In a septate uterus, the septum can be shaved off during hysteroscopy (metroplasty) to form a single uterine cavity without perforating the uterus
