Sigmoid volvulus

Presentation

Chronic constipation with acute onset of abdominal distension, nausea, and vomiting.

Patient Data

Age: 60 years
Gender: Male

Distension of the sigmoid colon with smooth tapering of its ends forming a closed-loop obstruction with a "coffee bean appearance" on coronal images. Twisting of the sigmoid mesentery and vessels giving the "whirlpool sign". No free intraperitoneal fluid or signs of perforation. The remainder of the large and small bowel are not distended.

Small simple renal cysts are noted bilaterally.

Sigmoid volvulus is the most common intestinal volvulus, seen in ~60% of cases and more common in elderly patients.
