Presentation

Abdominal discomfort.

Age: 40 years

Retroperitoneal adenopathy, also adenopathy within the mesentery with a large conglomerate surrounding the mid through distal mesenteric vasculature. Segmental circumferential soft tissue thickening of a loop of distal small bowel within the central pelvis, with some dilation of distal small bowel leading into this loop.

Case Discussion

Classic imaging findings of small bowel lymphoma, with mesenteric and retroperitoneal adenopathy, as well as a distal small bowel mass resulting in partial obstruction only despite circumferential involvement. The presence of characteristic adenopathy, particularly the lymph nodes encasing the mesenteric vasculature forming a conglomerate helps to increase confidence in favoring the correct diagnosis without tissue sampling first.
