Small bowel obstruction

Diagnosis probable

Presentation

Abdominal distention, pain, and constipation.

Patient Data

Age: 65 years
Gender: Male

Abdominal X-Ray

x-ray

Multiple distended gas-filled bowel loops with multiple air fluid levels arranged in a step-ladder pattern are observed in the central part of the abdomen, with a distal paucity of gas shadow.

ultrasound

Fluid filled dilated bowel loops ( >2.5 cm) with prominent valvulae conniventes are seen on ultrasonography.

Rapid reverse peristalsis was also noted during the USG scan.

Case Discussion

A 65-year-old male patient presented with constipation and pain in the abdomen. The patient had a previous history of abdominal surgery.

The patient was sent to the radiology department for ultrasonography. Multiple dilated bowel loops were found on ultrasonography. On plain x-ray of the abdomen in an erect posture, we found multiple air-fluid levels in the central abdomen with a relative paucity of bowel gas distally, confirming the diagnosis of intestinal obstruction.
