Presentation

Abdominal pain with left lateral ventral abdominal wall mass, for evaluation.

Patient Data

Age: 60 years
Gender: Female

There is evidence of left-sided sided wide neck bowel containing (sigmoid colon) lateral ventral abdominal wall hernia, located lateral to the rectus abdominis muscle, along the left linea semilunaris, with surrounding fat stranding and free air foci (gas in the hernial sac), with its neck measuring about 3.6 cm, the contained bowel shows multiple diverticula, with subcutaneous adjacent fat stranding.
Free air foci are also seen adjacent to the intra-abdominal part of the sigmoid colon.

Grade 1 anterior spondylolisthesis of L4 over L5 vertebral bodies.

Case Discussion

Spigelian hernia is a rare type of abdominal hernia.

In our case, there was acute diverticulitis complicated with perforation, involving the sigmoid colon and its herniated segment.

However, there is another less likely possibility of perforation due to a complicated Spigelian hernia, despite its wide hernial neck.
