Sternal fracture with retrosternal hematoma

Presentation

Patient presented to the ER after falling down 2 meters.

Patient Data

Age: 30 years
Gender: Male

Linear fracture seen in the lower midline sternum depression of the distal sternum with anterior displacement of the proximal portion of it with dense content retrosternally giving blood density suggesting hematoma with maximum thickness of 1 cm without definite extravasation.

Case Discussion

Sternal fractures occur in ~5% of blunt chest trauma with the manubrium being the most commonly injured part.

Associated injuries are common, occurring in ~50% (range 33-67%) of patients:

  • rib fractures, sternoclavicular joint dislocation

  • thoracic spine compression fracture

  • traumatic brain injury (TBI)

  • mediastinal hemorrhage, pneumothorax, hemothorax, cardiac tamponade, cardiac contusion, pulmonary contusion
