Subarachnoid hemorrhage due to ruptured anterior communicating aneurysm

Patient Data

Age: 60 years
Gender: Female

Diffuse arachnoid hemorrhage, centered within the anterior interhemispheric fissure and bilateral sylvian and interhemispheric fissures, with blood products extending inferiorly into the basal cisterns and to a lesser degree the prepontine cistern. Blood products are also present in the lateral, third and fourth ventricles with dilated ventricles.

Superiorly directed irregular appearing 7 mm aneurysm arising from the anterior commuting artery. The aneurysm has a narrow neck of approximately 2 mm.

Catheter angiogram confirms ACA aneurysm, best shown on oblique images.
