Presentation

Diminution of vision on right side.

Patient Data

Age: 40 years
Gender: Female
mri

Large saccular outpouching of supraclinoid right ICA is seen directed anterosuperiorly. It measures approx. 30 x 19.5 x 20mm (AP x TD x CC). It is compressing the intracranial segment of the right optic nerve, right basifrontal lobe and abutting the optic chiasma and left basifrontal lobe.

No evidence of subarachnoid hemorrhage is seen.

Case Discussion

Supraclinoid internal carotid artery (ICA) aneurysms commonly arise at the origin of the posterior communicating or anterior choroidal artery 1. Patients typically present with hemianopsia, which is seen in this case given the partial compression of right optic nerve.
