Suprasellar meningioma

Presentation

Visual disturbances.

Patient Data

Age: 50 years
Gender: Female

Large well-circumscribed lobulated suprasellar mass with intrasellar extension splaying the supraclinoid segment of the ICAs, compressing the pituitary gland inferiorly as well as the optic chiasma superiorly. It appears isointense to the cortical grey matter on T1 and T2, slightly hyperintense on FLAIR with a vivid relatively homogeneous enhancement on postcontrast sequences. No calcification or hemorrhagic component seen on the GE sequence. There is an extension to the interpeduncular cistern, elevating the floor of the third ventricle with no significant dilatation of the ventricular system.

Right parietal developmental venous anomaly (DVA) is noted.

Case Discussion

MRI features of a large vividly enhancing suprasellar mass compressing the optic chiasma with intrasellar extension compressing the pituitary gland most consistent with meningioma. Such case should not be mistaken for a pituitary macroadenoma.
