Suture granuloma

Case contributed by Mostafa Elfeky
Diagnosis almost certain

Presentation

History of abdominal exploration for complicated appendicitis 2.5 years ago, with a periumbilical swelling since then.

Patient Data

Age: 25 years
Gender: Male
ultrasound

A periumbilical hypoechoic subcutaneous area with a central wavy thread-like echogenic structure with parallel rail-like morphology measuring 1 cm, consistent with a foreign body, e.g. suture material. No intra-peritoneal extension, no hernia or fistula noted.

Case Discussion

The echogenic wavy linear structure most probably represents the retained suture material. The suture material often has a parallel hyperechoic "rail-like" morphology. The suture-induced inflammatory changes are referred to as a suture granuloma. Treatment is by excision of the granuloma and the retained suture.
