Tension pneumocephalus after septoplasty surgery

Presentation

The young male suffered from headaches and rhinorrhea 10 days following nasal surgery (septoplasty).

Patient Data

Age: 20 years
Gender: Male

In the scout view, there is a hypoattenuating area in the anterior portion of the skull vault.

CT scan showed air bubbles within the sulci, cisterns, and especially the ventricles.

There are bony defects in the sphenoid sinus roof.

Air-fluid levels are present within both the sphenoid and maxillary sinus.

Case Discussion

Pneumocephalus is the presence of air intracranially and can be identified easily by a CT scan.

CSF rhinorrhea and pneumocephalus are two rare, but potentially fatal intracranial complications of septorhinoplasty.

They are due to the consequences of skull base defects after septorhinoplasty surgery, a procedure regarded as an important but common procedure all over the world.
