Large right-sided pneumothorax with complete collapse of the right lung. Mediastinal shift toward the left, depression of the right hemidiaphragm and increased intercostal distances in the right hemithorax indicate tension.

Right pigtail pleural drain is positioned in the right midzone. The large right pneumothorax has reduced in size with partial aeration of the collapsed right lung. Mediastinal shift has resolved.
