Terminal ileal tuberculosis

Presentation

Chronic diarrhea and malabsorption for 3 months with bilateral lower limb swelling. Albumin levels were persistently low.

Patient Data

Age: 25 years
Gender: Female
ct

Smooth circumferential wall thickening of the terminal ileum. Cecum is preserved. Mild segmental dilatation of the distal ileal and proximal jejunal loops, however no definite bowel obstruction. 

Multiple subcentimeter mesenteric nodes. No necrotic nodes.

Dependent atelectasis in both lower lobes. No lung changes suggestive of pulmonary tuberculosis.

Case Discussion

The differential diagnosis for terminal ileitis in this patient includes inflammatory bowel disease, terminal ileal tuberculosis and vasculitides. 

Mycobacterium tuberculosis complex was detected from the peritoneal fluid MTB PCR of this patient, suggesting the diagnosis of terminal ileal tuberculosis.
