Presentation

Severe acute left scrotal pain.

Patient Data

Age: 17 years
Gender: Male
ultrasound

Left testis is seen swollen with heterogeneous texture. No blood flow on color and power Doppler.

Left spermatic cord is seen twisted giving the whirlpool sign.

Left epididymis appears swollen. Reactive hydrocele is seen.

Right testis appears normal, look at the comparison between right and left.

Case Discussion

This patient has come to the hospital with acute severe left scrotal pain. No history of trauma.

Ultrasound shows heterogeneous texture of left testis with absent blood flow on color and power Doppler. Concentric target like appearance of the left spermatic cord is noted. Power Doppler shows whirlpool sign which is typical feature of twisted spermatic cord.

Left orchiectomy was performed.
