Thalamic hematoma

Diagnosis almost certain

Presentation

Left-sided poor vision and right-sided leg weakness for 21 days.

Patient Data

Age: 45 years
Gender: Male
mri

There is a definable, intra-axial, altered signal intensity lesion that is centered on the left thalamus with extension into the left basal ganglia and left cerebral peduncle. Mass effects are seen as compression on the third ventricle with mild surrounding edema. Returning signals are hyperintense on T1, T2 weighted, and FLAIR images without definite post-contrast enhancement.

The rest of the brain parenchyma, bilateral lateral ventricles, and 4th ventricle are within normal limits.

7th and 8th nerve complexes are normal.

Visualized orbits are unremarkable.

No calvarial defect is appreciated.

Case Discussion

Current MRI findings are concerning for late sub-acute hematoma of the left-sided thalamic-ganglionic region with mass effects as described.
