Thyroid nodule for assessment.

Patient Data

Age: 45 years
Gender: Female
ultrasound

Mixed cystic solid, isoechoic right thyroid TR3 nodule measuring 4.9 x 3.9 x 4.2 cm.

Cytopathology Report

Fine Needle Aspirate Thyroid

MICROSCOPIC:

Prepared smears are adequate for assessment. There are follicular epithelial cells arranged in flat sheets, microfollicles and dispersed as there nuclei. There is focal fire flare and involutional change. There is oncocytic metaplasia. There is abundant colloid in the background. No architectural or cytological features of papillary thyroid carcinoma.

 DIAGNOSIS:

FNA thyroid: BENIGN (Australasian modification of Bethesda System); cytological features in keeping with benign adenomatoid nodule.

Case Discussion

Thyroid adenomatoid nodules are on the benign side of the follicular growth pattern spectrum that ranges from hyperplastic nodules to follicular carcinomas. They are a frequent cause of false positives for thyroid FNAs reported as follicular neoplasms.
