Transcatheter aortic valve implantation

Presentation

Known chronic asthmatic. Presents with non-resolving pneumonia. Known pacemaker patient.

Patient Data

Age: 80 years
Gender: Female

There is left ventricular cardiomegaly.

There is left subclavian access, external dual lead pacemaker in situ.

There is dense right middle lobe and lower lobe consolidation / obstructive pneumonitis.

A right hilar mass lesion is not excluded.

There is poor identification of the transcatheter aortic valve implantation or replacement (TAVI / TAVR) on frontal view and reverse frontal view too.

The lateral view confirms the TAVI, both on normal and reverse views.

Case Discussion

This is an example of a Medtronic CoreValve. This aortic valve is made from Nitinol (Nickel-Titanium alloy) and is MRI conditional. Patients can safely be scanned at 1.5 and 3 Tesla strength immediately after placement.

Expect MR image artifact if scanning the exact area of interest that includes the implanted aortic valve or scanning an adjacent area of interest.

(In this instance, the patient's external pacemaker was non-MRI compatible).
