Transfusion-associated circulatory overload

Presentation

Recent packed red blood cell transfusion with new crackles on auscultation and dyspnea.

Patient Data

Age: 80 years
Gender: Male

There are poorly defined but prominent interstitial markings bilaterally, consistent with diffuse interstitial edema. There is also thickening of the right horizontal fissure. However, there are no large pleural fluid collections.

There are findings suggestive of coronary artery bypass surgery as evidenced by the median sternotomy wires and the surgical clips.

Case Discussion

This is a case highly suggestive of transfusion-associated circulatory overload (TACO). The patient was saturating well on room air prior to the packed red blood cell (PRBC) transfusion. Shortly after completing the PRBC transfusion, the patient developed dyspnea and crackles on auscultation. A radiograph of the chest revealed findings suggestive of fluid overload and thus, TACO.
