Traumatic neuroma - ulnar nerve at elbow

Presentation

History of old trauma to the medial aspect of the elbow joint presented with pain and tenderness on the medial joint line.

Patient Data

Age: 40 years
Gender: Male

A defect within the cubital tunnel retinaculum measuring 5 mm in length with evidence of ulnar nerve fascicles outpouching through it measures 14 x 4 mm (yellow outline, image A).

Just distally there is partial nerve injury with loss of the medial portion of the nerve fascicles at the level of the cubital tunnel followed by focal nodular circumferential thickening of the ulnar nerve just distal to the cubital tunnel measures 7 x 8 x 11 mm with circumferential of about 33 mm2 (in comparison with the average circumferential diameter of about 8 mm2) denoting central traumatic neuroma.

Post-contrast images show no abnormal enhancement.

Case Discussion

Imaging findings demonstrate partial injury to the ulnar nerve at the level of the cubital tunnel associated with a focal defect in Osborn's ligament, herniation of the nerve fibers through this defect, and distal central traumatic neuroma formation.
