Tropical pulmonary eosinophilia

Diagnosis almost certain

Presentation

Cough and shortness of breath. No history of fever. Bilateral wheeze

Patient Data

Age: 15 years
Gender: Female

Multiple ill-defined tiny reticulonodular opacities in the bilateral lung fields, more in the central region.

CECT Thorax

Extensive tiny random nodules in the bilateral lung with diffuse ground glass opacities.

Few subcentimeter-sized discrete enhancing lymph nodes in the mediastinum and bilateral hila with no evidence of necrosis or calcification within it.

Case Discussion

The WBC count of this patient was 27,340/microliters with 62.6% of eosinophil. The IgE level was 7,751 KIU/L. The CRP, ESR and procalcitonin assay were normal. She had significant resolution of clinical symptom after start of steroid therapy.

So over all imaging findings and lab parameters are suggestive of tropical pulmonary eosinophilia/hyper eosinophilic syndrome.
