Presentation

Left pelvic pain and vaginal spotting.

Patient Data

Age: 30 years
Gender: Female

Left adnexal hemorrhagic lesion/hematosalpinx. The lesion shows clear cleavage planes with uterus and ipsilateral left ovary.

Case Discussion

Diagnosis was confirmed by laparoscopy and treatment with left salpingectomy.

Radiologists should be aware of the typical MRI appearance of ectopic pregnancy and should always take into account the potential for an ectopic pregnancy when a woman of childbearing age has a hemorrhagic adnexal mass or hemoperitoneum.

The diagnosis of ectopic pregnancy is typically based on a combination of quantitative assay for β-HCG and findings on pelvic sonography.

MRI features of tubal ectopic pregnancy include hematosalpinx, a hemorrhagic or heterogeneous adnexal mass, bloody ascites, and tubal dilatation and wall enhancement 1.
