Presentation

Sudden attacks of hematuria and diarrhea after a non-recent slip in the toilet, followed by gradual abdominal distention.

Patient Data

Age: 70 years
Gender: Male

A large encysted collection filled the major part of the abdominal cavity with air inside. The wall of the encysted collection is mildly thickening and blends inferiorly with the bladder wall. The bladder wall is deficient at the anterior portion of the bladder dome.

Incidental duodenal diverticulum also noticed,

Case Discussion

The abdomen was collapsed and re-distended after multiple evacuations of the collection were done via foleys and abdominal drains. The evacuated fluid is of a fixed amount (8 liters) each time of evacuation. A surgical intervention revealed a bladder tear that was repaired successfully.
