Presentation

Post-scrotal exploration now presents with a right para-testicular swelling, non-tender.

Patient Data

Age: 40 years
Gender: Male
ultrasound

The right and the left vas deferens are mildly engorged with clear fluid content. Focal medially disrupted/severed areas are demonstrated at the vas deferens (measuring approximately 0.80 cm in length on the right) post-exploration vasectomy.

Simple sub-centimeter thin-walled clear fluid-filled cysts eccentrically located at the right epididymal head, and at the left epididymal head, and body are noted. There are mild bilateral scrotal sacs fluids (<5ml) on either side. Both testicles are within the scrotal sacs and maintain their sizes, shapes, and parenchymal echo/vascular pattern.

Case Discussion

Symmetrical bilateral vasal ectasia with clear intraluminal fluid content with the dilations more distal on the right ductus deferens, and more proximal on the left vas deferens sequelae of known/confirmed case of an exploratory vasectomy. Occasionally, the epididymides may be thickened in such a setting however, in this case, no significant change is evident in the epididymides or the testes bilaterally, save for relative fluid-filled engorgement within the Vas deferens and the sub-centimeter, simple epididymal cysts (spermatoceles) changes chanced post vasectomy.

Postvasectomy complications including hematoma collections, abscesses, epididymal congestions, perineural fibrosis, extravasation of sperm, vascular stasis, painful sperm granulomas, lack of sperm granulomas, and circulating anti-sperm antibodies, have all been postulated and suggested 1.
