Vesicoureteric junction calculi in duplex ureter

Presentation

Referral for suspected malignancy due to visible hematuria.

Patient Data

Age: 75 years
Gender: Male

There is a duplex right collecting system with a bifid ureter. There are two distal right ureteric calculi with an 8mm calculus and a 12mm just proximal to the VUJ. No significant upper tract dilatation. Unchanged bosniak II right renal cyst.

Case Discussion

Duplex collecting systems are a rare embryological variant which predispose to urinary tract pathology. This patient was investigated for visible hematuria using a CT KUB to rule out calculi and a CT IV Urogram to rule out malignancy. The duplex right sided ureter is easiest to follow on the urographic phase images, however both images must be investigated to identified which ureter the stone(s) lie in.
