Vestibular schwannoma - intracanalicular

Presentation

Left tinnitus.

Patient Data

Age: 60 years
Gender: Female

Left-sided small intracanalicular lesion at porus acousticus with avid post-contrast enhancement. Axial thin cuts heavy T2 sequence (CISS) shows the lesion is continuous with the vesibulocochlear nerve (VIII), in keeping with vestibular schwannoma.

Case Discussion

As regards the location of the lesion, differential diagnosis includes intracanalicular acoustic schwannoma and meningioma. The continuity of the lesion with vestibulocochlear nerve (VIII) is in keeping with acoustic schwannoma. High resolution thin T2 sequence (e.g. CISS) is needed to detect small intracanalicular acoustic schwannomas which may be missed on conventional images.
