A large area of encephalomalacia changes surrounded by gliosis is seen in most of the left temporal lobe extending to frontoparietal and occipital lobes, causing ex vacuo dilatation of the ipsilateral lateral ventricle, suggesting old middle cerebral artery infarct.
A contiguous tract of gliosis is seen extending towards the deep cerebral structures, reaching the left side of the midbrain and pons, showing high signal intensity on T1WI, suggesting Wallerian degeneration.
