Wernicke encephalopathy

Diagnosis almost certain

Presentation

Worsening mobility for months. Rapid decline with quadriplegia, worse in the lower limbs, swallow difficulties, abnormal eye movements and confusion.

Patient Data

Age: 65
Gender: Male
mri

Symmetrical high T2/FLAIR signal in the peri-aqueductal region, bilateral medial thalami anteriorly, hypothalamus and mammillary bodies. Global supratentorial volume loss without regional or lobar preponderance.
