Presentation
Worsening mobility for months. Rapid decline with quadriplegia, worse in the lower limbs, swallow difficulties, abnormal eye movements and confusion.
Patient Data
Symmetrical high T2/FLAIR signal in the peri-aqueductal region, bilateral medial thalami anteriorly, hypothalamus and mammillary bodies. Global supratentorial volume loss without regional or lobar preponderance.
Hypothalamus. Mammillary bodies can be difficult to appreciate in the axil plane.

Classic changes of Wernicke encephalopathy. Mammillary bodies are part of the hypothalamus and involved in memory.
