Xanthogranulomatous pyelonephritis

The left kidney is enlarged with multiple dilated hypodense spaces. There is left-sided ureteric enhancement with a delayed nephrogram.
The calculus at the inferior pole of the left kidney is again demonstrated and nonobstructing.
Case Discussion
Xanthogranulomatous pyelonephritis results from chronic pyelonephritis and represents granulomatous disease in a non-functional kidney. There is usually associated renal calculi, formed by struvite calculi. Diabetes is a risk factor. This case demonstrates the bear paw sign.
