What injury does a "Fallen lung" indicate?
Injury to the tracheobronchial tree. When there is complete transection of a bronchus, the lung on the side of the bronchial injury may fall posterolaterally away from the hilum (“fallen lung” sign). On CT the lung is seen posteriorly as the patient is supine.
What is the significance of a sentinel clot?
The ‘sentinel clot sign’ is an important sign to look for on CT to help localise the site of haemorrhage. On CT, acute clotted blood will appear hyperdense compared to surrounding acute unclotted blood or more chronic haemorrhage. In the case of splenic injury, a sentinel clot sign can indicate splenic injury even in the absence of a visible laceration.
What structure is most at risk of being injured when a hepatic laceration extends to the porta hepatis?
The bile duct.
