What is the finding?
There is an osseous projection from the inferior surface of the acromion process.
What does this finding most likely represent?
Ligamentous ossification, most likely of the coracoacromial ligament. It may also represent a large osteophyte, but it is not extending from the acromioclavicular articular margin. It is not likely to be an osteochondroma as it is too pointed and in an unusual location.
Are there any ways to determine rotator cuff tears on radiograph?
Assessing the subacromial space can give a gross assessment of the superior rotator cuff. If the humeral head contacts inferior acromion with superior migration of the humeral head, there will be a full-thickness supraspinatus tear.
