Large nontraumatic bilateral pneumothoraces are uncommon.  No mediastinal shift is evident as the bilateral pneumothoraces equilibrate any shift.

The chest cannot be totally covered on the image, but the supine projection indicates bilateral deep sulci, a typical feature in this projection.
