The MRI sequences demonstrate engorgement of the right cavernous sinus, dilated ipsilateral superior ophthalmic vein, and an abnormally dilated flow voids vascular structures in the anterior middle cranial fossa, indicating a caroticocavernous fistula (CCF).
On MRA the presence of high flow communication between the intracavernous portion of the right ICA and the cavernous sinus is seen, indicating most likely a direct CCF.
