Perforated sigmoid diverticulitis with forming abscess. In addition to intraperitoneal air, there is the surprising find of air dissecting into the retroperitoneum and mediastinum. This was managed with urgent resection of the sigmoid colon with end colostomy (Hartmann procedure).
