What is the most common underlying connective tissue disease leading to NSIP?
Systemic sclerosis.
There are multifocal intralobular and interstitial inflammatory ground glass densities, interlobular septal thickening, and microcystic honeycombing patterns in both lungs more prominent in the middle to lower zone and basal predominance and more in the periphery of the lungs with immediate subpleural sparing in some parts. Note also the remarkably distended thoracic esophagus. No significant mediastinal lymph nodes are seen.
