The patient in this case had known Thalassemia major and a history of multiple blood transfusions. With the patient's presentation of shortness of breath and decreased hemoglobin level, there were initial suspicions of lung pathology.

Hepatosplenomegaly turned out to be the incidental finding on this otherwise normal chest x-ray.
