What are the bony abnormalities in the chest X-ray?
Fractured left clavicle. Multiple rib fractures on the left, 3rd to 7th ribs.
What other abnormalities are seen?
Loss of the left hemidiaphragm with marked opacification of the left mid and lower zones. The left costophrenic angle is not identifiable. The left main bronchus is elevated, suggesting left upper lobe collapse.
Given the clinical history and chest xray findings, what is the most likely cause of the appearance of the left hemithorax?
Given the history of trauma and multiple fractures, a large left sided haemothorax would be most likely.
Poor inspiratory effort and rotated to the left. ETT in good position. Multiple rib fractures noted in the left 3rd, 4th, 5th, 6th and 7th ribs. Fracture of the infero-lateral aspect of the left scapula. Increased opacity of the left lung, more pronounced in the left lower zone, most likely indicating collapse with associated pleural effusion.
