This is a case of a newborn female baby presented with bilateral inguinal region swelling increasing slightly in size upon crying of baby. Ultrasound of the palpable swelling in bilateral inguinal region shows bulging of gut loops upon Valsalva/crying through a defect in bilateral inguinal region measuring approximately 7-8 mm consistent with bilateral canal of Nuck hernia.

No herniation of ovaries noted in this case.

Canal of Nuck hernias are indirect inguinal hernias in which the fluid or abdominal contents like bowel, omental fat, ovaries, fallopian tubes or, rarely, urinary bladder or uterus protrude through the deep inguinal ring lateral to the inferior epigastric vessels. The herniation takes place due to patent embryonic processus vaginalis. In females, the patent processus vaginalis is called canal of Nuck.

Two major abnormalities associated with patent processus vaginalis are hydroceles and hernias.

Canal of Nuck hernias containing ovaries should be recognized earlier due to increased risk of ovarian torsion without surgical intervention.
