Pylorospasm

Discussion:

This is a case of a neonate diagnosed with infantile pylorospasm based on the clinical and radiological criteria. Congenital hypertrophic pyloric stenosis (CHPS) is one of the most important causes of gastric outlet obstruction in neonates. Infantile pylorospasm is a strong mimicker of CHPS. Both conditions show increased thickness of the pyloric muscle layer and elongated pyloric canal. Unlike infantile pyloric stenosis, which shows persistent gastric outlet obstruction, infantile pylorospasm shows intermittent obstruction. The infantile pyloric stenosis shows the string sign in the upper gastrointestinal contrast study, while infantile pylorospasm exhibits free passage of contrast. The diagnosis was confirmed as the patient improved clinically in the following days.
