The CT features are most consistent with hepatic hydatid cysts.

The liver is the most common site of hydatid infection. The WHO has classified hepatic hydatid cysts into various stages, and the water lily sign is visible in stage CE3a. This phase represents a transitional period between active and inactive disease. The membrane detachment is caused by several factors, such as decreased intracystic pressure, degeneration, host response, trauma, or therapy.
