Hyperenhancing pituitary microadenoma

Presentation

Galactorrhea, significantly increased serum prolactin levels.

Patient Data

Age: 30 years
Gender: Female

The study reveals a focal lesion, measuring approx. 3 x 2 x 1.5mm (AP x TD x CC) in anterior pituitary towards right of midline. It appears hyperintense on T2, iso to mildly hypointense on T1 shows hypoenhancement in early phases of dynamic contrast enhancement with hyperenhancement in delayed phases.

Case Discussion

Without prolactin levels it is difficult to know if this is a secreting adenoma (most prolactinomas present as macroadenomas) or some other lesion. See elevated prolactin.

Delayed contrast enhancement of pituitary microadenomas is variable, ranging from hypoenhancement (most common) to isointense to the rest of the gland, to hyperenhancement (retained contrast). This shows the uncommon hyperenhancement of pituitary microadenomas on delayed contrast-enhanced images.
